Health Care Survey
Help us break the silence around violations of your Economic Human Right to adequate health care. WEAP has created this health care survey to let you speak out about your own personal experiences with the US health care system. Why? Because workers all across the country, employed and unemployed alike, are finding it harder and harder to get decent health care. With this survey you can break the silence by telling your story and making your voice heard! The survey only takes a few minutes to complete and the results will be publicized in order to help expose the millions of Economic Human Rights violations occurring daily. To take the survey, you can do one of the following:
1. For the first time, you can conveniently take our health care survey on-line by clicking here!
2. Or you can download our Economic Human Rights Violation Report Form and Health Care Addendum here. Fill these survey forms out and then mail or fax them into our office.

"We, the poor, jobless, downsized, uninsured victims of welfare reform and others abused by the institutions of domination are no longer silent. We are moving forward with the legacy of Dr. Martin Luther King, Jr., Fannie Lou Hamer, and so many freedom fighters to improve the lives of Americans."
-Portia Anderson, WEAP
